Fighting COVID-19 — All hands on Deck

COVID-19 has presented a significant challenge to all healthcare organizations, large and small. Maintaining appropriate staffing levels and ensuring patients have access to the necessary therapies are just a couple of examples of the issues they face.

Big problems present big challenges. Big challenges present big opportunities — Opportunities to work together, as a community, to bring about a better outcome for everyone. A viable vaccine produced in months rather than years. Treatment protocols with life saving monoclonal antibody treatments, launched in record time, utilizing the Emergency Use Authorization (EUA) process. The time calls for all hands on deck.

While there is a need to expedite treatment in times like these, we can’t compromise on patient safety – And RxTOOLKIT® is here to help.

Currently the FDA has introduced three COVID-19 vaccines and three monoclonal antibody drug therapies (though one has since been revoked). RxTOOLKIT has kept abreast of the new drug launches (as well as the frequent updates that have occurred) and immediately launched RxWORKFLOW for IV Safety™ drug monographs for the monoclonal antibody drugs, as well as RxWORKFLOW for Vaccine Safety drug monographs for the approved COVID-19 vaccines. We also launched drug-specific courses to train and track ongoing staff competency for these therapies in RxTOOLKIT eLEARNING™.

In addition, collaborating with the National Infusion Center Association (NICA) and the US Department of Health and Human Services COVID-19 Monoclonal Antibodies Therapeutic Team, we have created several new RxWORKFLOW Drug Preparation and Administration Worksheets to be provided to their users and to be featured as part of our comprehensive clinician support resource, RxWORKFLOW for IV Safety.

These worksheets are designed to establish a consistent process by including automatic dose and administration calculations and step-by-step instructions for chairside/bedside administration of these new lifesaving drugs.

As industry leaders in providing tools that emphasize patient safety and the importance of a standardized process to the infusion therapy provider community, we also wanted to make certain ALL providers, clinicians, and patients could benefit from these medication safety tools.

Are You Ready to Prepare and Administer COVID-19 Vaccines and Therapies?

The country is now entering a new phase of COVID-19, one that is less reactive and more proactive in the form of prevention and treatment.

As a medical community, we are now being armed with tools to fight the COVID pandemic. Science is working on prevention and treatment at the same time. Prevention means vaccines. Treatments include monoclonal antibodies.

Healthcare providers now have access to a new COVID vaccine from Pfizer through an Emergency Use Authorization (EUA) and soon will have one from Moderna, with more in the pipeline coming soon. Additionally, the FDA has already granted EUA’s to two monoclonal antibody COVID therapies for use in a non-acute setting, Bamlanivimab from Eli Lilly and the Casirivimab/Imdevimab combination from Regeneron.

We, at RxTOOLKIT, want you to be prepared. Our RxWORKFLOW™ monographs:

Introducing RxWORKFLOW for Vaccine Safety™

While we are all facing the challenges of this pandemic and with one of the primary focus points being the development of a vaccine, we at RxTOOLKIT® are excited to announce a new RxWORKFLOW™ collection that will be invaluable to anyone who administers vaccines.

Did you realize that there are 85 different vaccine products approved for use by the FDA? While some of them are reserved for use by the military or only in emergency situations, there are 67 that are available for use on a non-emergency basis.

In addition, vaccines are a major cause of medication errors as reported by the Institute of Medication Practices (ISMP). Among the reported findings*:

  • Most reported errors with vaccines reach the patient, as opposed to being a near miss or hazardous condition.
  • Most of the reported errors occurred in medical clinics, physician practices, hospital (ambulatory), and public health immunization clinics.
  • The four most commonly reported types of errors involving vaccines were: wrong vaccine administered, wrong age (i.e. patient not correct age for vaccine being given), extra dose, and expired vaccine.
  • More than half of the events involving administration of the wrong vaccine were associated with similarities in vaccine names, abbreviations, or labeling/packaging.

Tools to Support Success

For the infusion center nurse, not being familiar with a drug they need to prepare and administer can be a source of stress and anxiety.

This is an experience I am personally familiar with. At one point early in my career, I received an order for a drug called ELAPRASE (idursulfase). I knew nothing about this drug at that time. How do I prepare it? How is it administered? What instructions do I give the staff asking me questions? What concentration do I prepare?

The experience made me feel vulnerable and honestly pretty stressed out. I realized in that moment, that I desperately needed a quick resource to help me determine the answers to my questions about this drug and allow me to provide accurate directions to my staff.

This moment was the birth of RxWORKFLOW™ – an operationally integrated reference tool that includes the critical information and tools needed to dose, prepare, and administer any infusion drug.

In the beginning, however, I was still using my trusty handheld calculator to do the math necessary to complete the tasks at hand. I, like everyone else, have fat fingers and the vulnerability of using this tool became more and more apparent. That’s when we decided to build RxQuickCALC™, RxCALC™, and RxDoseCHECK™ and integrate them right into RxWORKFLOW™.

These drug specific calculation tools are designed to minimize user input and provide accurate calculations. This is especially important when infusion nurses need to calculate dose specific taper steps for a drug like RITUXAN (rituximab). Without a tool like this, nurses will often need to call the pharmacy (or coworker) to double check their math, then write down the taper steps on a piece of paper, and tape that paper to the IV pole where the medication is hanging—Certainly not the safest or most efficient method!

Following the premise that consistency promotes safe practices and predictable patient outcomes, we have also recently introduced enhancements to our RxCALC™ preparation and RxCALC™ administration pop up calculators.

Not only do the calculators provide the math for dose preparation, they also provide step-by-step procedural information to help ensure consistent medication preparation. This provides the nurses a consistent method of administering all drugs, especially those with some complex administration calculations such as rituximab, infliximab, or any of the immune globulins.

Our new RxCALC™ for Dose Administration follows the same format and includes many specific weight-based dose calculators that incorporate these same features. For the free standing infusion nurse, we provide a consistent procedure for preparation and help to insure the details such as needle size or filtration requirements are not forgotten.

When these tools are integrated into an operation’s day-to-day policies and procedures, it significantly improves patient and staff safety, efficiency, consistency and staff confidence, even for those with minimal or outdated experience. Reflections on the Future of Infusion Safety

In life, you don’t often get a chance to be a part of something at its inception. Those of us who attended the first NICA meeting in Austin were provided just such an opportunity. For the very first conference put on by NICA, it was flawless. The breakout sessions were informative and subject matter was on point. The food was even good! Credit goes to NICA leadership: Brian, Morgan, Amy and the rest of the team.

For the RxTOOLKIT team, the best part of the meeting was the people: the attendees, the speakers, and the other exhibitors. We were fortunate to be part of many lively and productive discussions about issues facing the infusion market today: medication safety, operational costs, quality of care, clinical excellence, patient satisfaction, and staff training.

What became clear to us was that for providers, the patient and their experience was one of the most important goals of care. To accomplish this the focus was on staff competency, appropriate technology, and outcome measurement. This included using outcome data as a tool to rewrite and improve the very goals of care.

Another big highlight of the meeting was the release of the NICA Minimum Standards for Outpatient Infusion. This important document is the first step in developing the process and consistency necessary for the safe preparation and administration of sterile parenteral products in an outpatient infusion environment.

We are also thrilled to report that RxTOOLKIT already has tools available that help providers meet or exceed a number of the recommendations made by the NICA Minimum Standards! We are excited to be an integral part of helping this vital industry in providing a safer, more convenient, and cost-effective channel for infusion therapy in patients with chronic illness.
